ARRANGEMENT OF INURNMENT SCHEDULE


In the Philippines, people are now more open-minded regarding cremation. And one of the most important parts of it is the inurnment. Inurnment is the placement of urn (cremated remains) into the columbarium unit while interment is the ceremony of burying the body remains in memorial lots.


This blog offers an overview of the steps you should follow to arrange the inurnment schedule properly.


SECURE ALL THE NECESSARY DOCUMENTS

Below is the list of requirements:

1. Copy of Death Certificate

2. Copy of Certificate of Cremation

3. Burial Permit – the family must obtain this from Health Department of the City Hall where the memorial park is located. Please take note that if the remains was cremated from other city, transfer permit must be obtained from the city hall where the crematorium is located.

4. Authorization Letter from the Lot/Columbarium unit owner – the lot owner must provide authorization letter which states that he/she allows the interment of the deceased. A copy of valid ID with 3 signatures must be attached along with the copy of the Certificate of Ownership or Official Receipt of full payment.

5. Consent Letter of the Closest Kin

a. If the deceased is married and the spouse is alive:

· Copy of Marriage Contract

· Consent letter or Affidavit of consent of the spouse that she/he gives her/his consent that the remains of the deceased be buried in MMP/HCMP.

· copy of valid ID with 3 signatures

b. If the deceased is married and the spouse is deceased:

· Copy of Birth certificates of living children (proof of relation)

· Consent letters of all living children

· copies of valid IDs with 3 signatures

c. If the deceased is single:

· Consent letter or Affidavit of Consent of the closest kin (usually the parent of the deceased, if the parents are deceased, closest kin will be the siblings)

· copy of valid ID with 3 signatures


BRING THE DOCUMENTS TO THE SERVICING SECTION OF THE PARK OFFICE FOR THE ARRANGEMENT

Each park offices has Servicing Section. As soon as you arrived at the park office, just inform the guard that you need to arrange

the inurnment schedule.


GET THE PAYMENT ORDER FORM AND PAY THE INURNMENT FEE

The columbarium inurnment fee is 1,500.00. Engraving in the marker is not included. Additional 2,500 must be paid for the engraving of name. The fee for inurnment in lawn lots or any other garden lots is different.



IMPORTANT NOTE: Arrangement must be done 3 days prior the inurnment schedule.
